This parallel-group randomized clinical trial, published in JAMA Internal Medicine, is the Barcelona substudy of the PREDIMED trial -- a long-running Spanish nutrition intervention testing whether the Mediterranean diet, supplemented with extra virgin olive oil or mixed nuts, can change cardiovascular and cognitive outcomes in older adults at high vascular risk. Across a median follow-up of 4.1 years in 447 cognitively healthy volunteers, the Mediterranean diet plus extra virgin olive oil arm scored significantly better on global cognition and frontal cognition composites compared with a low-fat control diet, with the control group showing significant cognitive decline from baseline across every composite measured. The study is part of the broader evidence base behind olive polyphenols and brain health.

Why This Study Matters

By 2015, observational evidence had repeatedly linked the Mediterranean diet to slower cognitive decline. But observational studies cannot rule out the obvious confounder: people who eat Mediterranean diets in their 60s and 70s also tend to be more educated, more physically active, and wealthier. Whether the diet itself -- or the lifestyle that accompanies it -- was driving the cognitive signal remained an open question.

The PREDIMED trial was built to answer that question with a randomized design. The Barcelona substudy reported here took 447 of those PREDIMED participants -- all cognitively healthy at enrollment, all at high cardiovascular risk (with type 2 diabetes or at least three CVD risk factors) -- and added a full neuropsychological battery at baseline and at the end of follow-up. The three arms were identical to the main PREDIMED protocol: Mediterranean diet plus one liter per week of extra virgin olive oil, Mediterranean diet plus 30 g/day of mixed nuts, or a low-fat control diet (the prevailing dietary advice of the era).

This design isolates the dietary intervention from the lifestyle baseline. Three randomized arms, the same population, the same clinic, the same testing protocol -- the only systematic difference is what people ate. If olive oil delivers a cognitive benefit independent of lifestyle, this is the kind of trial that detects it. The Barcelona substudy is a distinct PREDIMED cognitive analysis from the Martinez-Lapiscina 2013 PREDIMED-NAVARRA paper, which used different cognitive instruments at a different study site.

How It Was Designed

The basics are in the study design bar above: 447 participants enrolled, median 4.1 years of follow-up, three diet arms, Hospital Clinic de Barcelona, ISRCTN35739639.

A few design choices establish credibility. The trial was parallel-group randomized -- participants were assigned to one of three diet arms and stayed in that arm for the duration. The EVOO arm was given one liter per week of free EVOO, enough to fully supply household cooking and dressing needs for one adult; the nuts arm received 30 g/day of mixed nuts (walnuts, almonds, hazelnuts); the control arm received written and verbal advice to reduce dietary fat, the standard-of-care intervention at the time.

Cognitive function was assessed with a six-instrument neuropsychological battery: Mini-Mental State Examination, Rey Auditory Verbal Learning Test (RAVLT), Animals Semantic Fluency, Digit Span from the Wechsler Adult Intelligence Scale, Verbal Paired Associates from the Wechsler Memory Scale, and the Color Trail Test. Individual test results were converted to z-scores and combined into three composites -- memory, frontal cognition (attention and executive function), and global cognition -- so that the analysis would not be dominated by any single instrument's idiosyncrasies.

Follow-up cognitive tests were available in 334 of the 447 enrolled participants (75%). Multivariate analyses adjusted for age, sex, education, APOE genotype, vascular risk factors, depression, and family history of cognitive impairment. The trial was pre-registered on the ISRCTN registry, locking in the cognitive outcomes and the analytic plan before unblinding.

What They Found

Across all three cognitive composites, the control diet group showed significant decline from baseline. The two Mediterranean diet arms held steady or improved.

Reading the Results

The cognitive composites group into three areas worth pulling apart.

Global cognition. The composite that combines every test in the battery is the headline. The EVOO arm finished 0.43 standard-deviation units above the control arm (+0.05 versus -0.38), a difference significant at p = 0.005. In plain terms: control-arm participants showed measurable cognitive decline over four years of normal aging, and EVOO-arm participants did not. The intervention did not appear to enhance cognition in absolute terms so much as it prevented the decline that the control arm experienced.

Frontal cognition (attention and executive function). This is the composite most sensitive to vascular contributions to cognitive aging -- the executive function domains that depend on the integrity of small blood vessels supplying the prefrontal cortex. The EVOO arm gained 0.23 z-units; the control arm lost 0.33; the gap was significant at p = 0.003. This was the largest effect in the study, and it fits the proposed mechanism: olive polyphenols reduce vascular inflammation and oxidative damage, which preserves the cerebrovascular bed that frontal executive function depends on.

Memory. Both Mediterranean arms outperformed the control on the memory composite, but the nuts arm produced the strongest signal (p = 0.04 versus control). The EVOO arm's memory composite was favorable but did not reach statistical significance versus control on this domain alone. The two Mediterranean interventions are not interchangeable -- nuts and EVOO appear to drive different cognitive subdomains, plausibly through different mechanisms (omega-3 and alpha-linolenic acid from walnuts; polyphenols and monounsaturated fat from EVOO).

Individual tests. RAVLT (a verbal learning task) and Color Trail Test part 2 (a divided-attention task) each showed independent significant improvements in the EVOO arm versus control, mirroring the composite-level pattern.

What Didn't Change

The two Mediterranean diet arms were not significantly different from each other on any composite. Both outperformed the low-fat control diet, but the comparison between EVOO and nuts is not the contrast this study powered. Mini-Mental State Examination scores in isolation did not differ across arms either -- the MMSE is a coarse instrument designed to detect overt dementia, not subtle age-related decline, so the absence of a signal there is consistent with the rest of the data.

The trial cannot establish that EVOO would produce the same cognitive benefit in cognitively impaired adults, in younger populations, or outside the broader Mediterranean dietary pattern. The intervention was always EVOO plus a Mediterranean dietary context, not EVOO in isolation.

Broader Context

This Barcelona substudy is one of two PREDIMED cognitive analyses published in the early 2010s. The earlier Martinez-Lapiscina 2013 PREDIMED-NAVARRA paper reported a 66% lower risk of mild cognitive impairment in the EVOO arm over a separate Navarra-region cohort. The Barcelona analysis published here measures a different outcome -- rate of cognitive change on a continuous neuropsychological battery -- and reports it in a separate Catalan cohort. Together, the two PREDIMED cognitive substudies establish a coherent signal: randomized assignment to a Mediterranean diet supplemented with extra virgin olive oil produces measurably better cognitive trajectories than the low-fat dietary advice that was standard at the time.

The 2011 European Food Safety Authority opinion authorized a health claim for olive oil polyphenols and protection of blood lipids from oxidative damage at 5 mg of hydroxytyrosol and derivatives per day. The plausible mechanism linking that authorized cardiovascular claim to the cognitive outcomes seen here runs through the cerebrovascular bed: oxidized LDL drives endothelial dysfunction; endothelial dysfunction reduces cerebral perfusion; reduced perfusion accelerates age-related cognitive decline. PREDIMED-Barcelona is consistent with that mechanism without proving it.

The 2018 PREDIMED republication in the New England Journal of Medicine reaffirmed the cardiovascular finding -- a 30% reduction in major cardiovascular events with Mediterranean diet plus EVOO -- after the original PREDIMED was retracted and reanalyzed for randomization concerns. The cognitive substudies were re-examined alongside the main analysis and held up under the corrected statistical treatment.

Olivea's Dosage

This trial used approximately one liter per week of extra virgin olive oil -- roughly 50 mL per day, or about 3 to 4 tablespoons. A single tablespoon of Olivea extra virgin olive oil supplies a meaningful fraction of that intake.
